The forms below have been provided for APRS member convenience. We ask that you please print out the form, fill it out and sign it, then mail the form via regular mail, hand deliver or fax it to the Pension Office. We must have a signature on each form in order to for the document to be valid. Emailed forms will not be accepted.

PLEASE NOTE: IF YOU ARE AN ACTIVE POLICE OFFICER, YOU SHOULD BE SUBMITTING YOUR ADDRESS CHANGES TO THE HUMAN RESOURCES OFFICE AND NOT THE PENSION OFFICE.
